Unless you are entering the hospital through emergency services, the first person you meet will be the admitting clerk. You provide the clerk with information that is a necessary part of your medical record. During your stay, test results, treatments and records of your daily progress also become a part of this medical record. Rest assured, all information is kept confidential. A copy of this record will be made available only with your written permission.
